Iedereen ontvangt op hetzelfde moment dezelfde informatie, zo zijn afspraken duidelijk en voorkom je misverstanden. - Afspraken: Plan afspraken of MDO's met de zorgontvanger en nodig iedereen uit die de zorgontvanger kan ondersteunen vanuit haar expertise. - Thuismetingen: Deel meetwaarden, zoals gewicht, bloeddruk, saturatie, ACQ score of CCQ score. - Diabetes beleid: Verstuur dagcurves en pas gemakkelijk het insulinebeleid aan. - Uitvoeringsverzoek: Vul het uitvoeringsverzoek, deel en onderteken het digitaal - Wondplan: Maak een wondplan aan om de voortgang van (chronische) wonden bij te houden, door foto's te delen.
